Sometimes, something happens in a research or teaching lab, and you need to report it. But what should be reported, and where does it go?

Near-Miss: When there was a Potentially Hazardous Situation, something that could have resulted in an accident or injury, but did not

• Report using the "Safety Solutions" form, with the term "Near Miss"

Incident: Something that resulted an injury or illness of a UI community member (Student, Volunteer, TA, Staff, Faculty, etc.)

• Report using the "Incident Report" form

Safety Solutions: When you notice safety hazards or concerns, or have suggestions to improve safety culture, procedures, or oversight

• Report/Suggest using the "Safety Solutions" form
