Safety solutions is a mechanism to report: 

• safety hazards or concerns, 
• suggestions to improve safety, including the safety culture, procedures, or oversight 
• near-misses = a situation that could have resulted in an incident or injury but did not. 

Should be submitted within 24 hours of the near-miss occurring/witness of the safety concern
